Pauline Campbell was found dead on 15th May 2008, lying on the grave of her daughter Sarah. Sarah tragically died in January 2003 whilst in HMP Styal.

4WardEver UK set up a website in memory of Pauline shortly after her death was announced, and maintains this site in her honour and to ensure that stories related to the deaths of women and young girls within the prison and psychiatric system are kept in the public eye.

Mortuary managers face prosecution after they gave custody death family the wrong body

Three mortuary managers could face prosecution after missing 10 chances to realise they had given the family of a paratrooper who died in custody the wrong body for burial.

Christopher Alder’s relatives unwittingly buried the body of a 77-year-old woman in 2000 but 11 years later it was realised there had been a mistake.

The error was only discovered when the Nigerian family of Grace Kamara arrived to prepare her body for burial in November 2011.

Police constables cleared of allowing mentally ill man to die in front of them

Two police officers have been found not guilty of allowing a mentally ill man to die in front of them after detaining him in his home.

PCs Maurice Leigh and Neil Bowdery, of Kent Police, both denied misconduct in public office over the death of paranoid schizophrenic Colin Holt.

Mr Holt, 52, died from positional asphyxia at his flat in Gillingham, Kent, on August 30, 2010 after the officers were called to return him to hospital under a Mental Health Act order.
